Paraplegia due to New Onset Spinal Metastasis during Pain Treatment of Metastatic Renal Cell Carcinoma

Abstract
A 60-year old man with renal cancer in whom metastasis to the rib was found in 3 months ago underwent radiation therapy. He had right chest and left mid thoracic pain for one month and received medical treatment at thoracic surgery department. The metastatic site was both sixth ribs. Pain was not relieved at all. So he was transferred to the pain clinic. Intercostal nerve block and trigger point injection were performed. After then, the patient became satisfied with almost painless state. After nineteen days, he visited again with paraplegia. Magnetic Resonance Imaging (MRI) was performed and spinal cord compression due to metastasis to the 5th thoracic vertebrae was confirmed. A very careful approach may be needed because rapid metastatic cancer such as renal cell carcinoma may cause rapid metastasis to the skeletal system such as the spine.
